Job summary

BAYADA Home Health Care is seeking a Physical Therapist for weekend home health visits in the Tucson area. The role supports adult and geriatric clients across Pima and Pinal counties on a per-visit basis.

Candidate must hold an AZ PT license with at least one year of PT experience. Independent work ethic, strong interpersonal skills, and EMR proficiency are essential to provide high-quality rehabilitative care at patients' homes.

Qualifications

• Current AZ Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from an APTA/CAAHEA accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and problem-solving skills.
• EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance).
• Implement wound care interventions and dressings.
• Develop and implement individualized care plans (therapy exercises, gait training).
• Reassess and revise care plans with nursing in the multidisciplinary team.
• Educate clients and family on rehabilitative care and home safety.
• Document observations and interventions in the EMR.
