Job summary

BAYADA Home Health Care in Tucson, AZ is seeking an experienced Physical Therapist (PT) for a full-time role delivering in-home visits across Pima County. You will join a multidisciplinary team providing rehabilitative care and skilled nursing to clients at their residences and in senior living communities.

Qualifications

• Arizona PT license required.
• Minimum of one year of PT experience.
• Graduation from a program approved by the American Physical Therapy Association (APTA) or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, strength, gait, safety).
• Implement wound care interventions with evidence-based dressings and topical agents.
• Develop and implement individualized care plans (manual therapy, gait training, balance).
• Continuously assess and revise the care plan with nursing in the multidisciplinary team.
• Educate clients, family members, or representatives in rehabilitative care and activities.
• Accurately document observations and evaluations in the EMR.
