BAYADA Home Health Care is seeking a Physical Therapist to provide weekend home health visits in the Phoenix area. You’ll build meaningful patient relationships, work with autonomy, and receive strong clinical mentorship and onboarding support.

Qualifications

• Current Arizona PT license (or eligibility).
• Graduation from an accredited PT program.
• Comfort with EMR systems (training provided).

Responsibilities

• Perform mobility, strength, balance, and safety assessments.
• Develop individualized, evidence‑based treatment plans.
• Provide gait training, manual therapy, balance work, and therapeutic exercise.
• Implement skin and wound‑prevention strategies.
• Educate clients and families to promote long‑term success.
• Document care using our intuitive EMR system.
