UnitedHealth Group's Caretenders division is seeking a Physical Therapist in Home Health to assess and evaluate patient needs related to mobility, endurance, pain, and functional status, then help plan and implement a treatment program.

It will involve providing interventions to improve well-being, documenting progress, and collaborating with the health care team while following state guidelines. A $20,000 sign-on bonus is offered for external applicants.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Evaluates the patient's functional status and physical therapy needs; assists physician in plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports patient response to treatment and any changes to supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient, family and care team about plan of care, limitations and progress.
• Visits patients per Plan of Treatment; completes progress notes and submits per policy.
• Develops PT plan of care within state guidelines; reports outcomes and frequency of care.
