Job summary

UnitedHealth Group, through Caretenders, is seeking a Physical Therapist for Home Health. You will assess patient needs, develop a therapy plan, implement interventions, and evaluate progress across varied home settings.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and needs to inform therapy plan.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Informs patient and family about plan of care, limitations, and progress.

Education

Current Physical Therapy licensure

Current CPR certification (preferred)
