AHN Healthcare@Home is hiring a Weekend Admissions Registered Nurse (RN) to provide skilled nursing care to patients in their homes around Leechburg, PA. This role offers a substantial bonus potential and opportunities for growth in a recognized home health leader.

You will perform start-of-care assessments, coordinate admissions, educate patients and families, and ensure accurate EMR documentation while traveling within your territory.

Qualifications

• Active RN license in the state you work in.
• Minimum of 1 year RN experience in home health or similar setting.
• Current driver’s license and ability to spend ~20% of your day driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Conduct start-of-care assessments, clinical evaluations, medication reconciliation, and initial care plans.
• Educate patients and families about admission, diagnoses, and treatment plans.
• Coordinate admissions with strong clinical judgment and compliant EMR documentation.
• Initiate evaluations with other disciplines when indicated.
• Travel to patient homes within assigned territory and work independently yet with team support.
