Active PA RN license, at least 1 year of RN experience, and a valid driver's license are required. Expect travel within the assigned territory; a $10,000 first-year success bonus is offered, along with medical/dental/vision, PTO, 401(k),

Qualifications

• Active RN license in Pennsylvania.
• One year of RN experience in home health or similar setting preferred.
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in home health settings.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management.
• Document care in EMR and ensure compliance with procedures.
