Job summary

AHN Healthcare@Home is seeking a Weekend Admissions Registered Nurse (RN) to provide skilled nursing care to patients in their homes in the Erie, PA area. This role includes a $10,000 first-year success bonus plus a $10,000 sign-on bonus, with rewards extending beyond the first year.

Responsibilities include comprehensive start-of-care assessments, medication reconciliation, developing initial care plans, and coordinating with an interdisciplinary team.

Qualifications

• Active RN license in the state you work.
• One year of RN experience delivering care in home health.
• Current driver’s license and ability to drive ~20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders.
• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one.
• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation.
• Initiate evaluations to other disciplines when indicated.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
