AHN Healthcare@Home in Canonsburg, PA is seeking a Weekend Admissions Registered Nurse (RN) to provide skilled nursing care in patients' homes. You will join a trusted home health team delivering autonomous, patient-centered care with meaningful patient relationships.

This role requires start-of-care assessments, care planning, patient education, and accurate EMR documentation, with travel within the assigned territory. A $10,000 first-year success bonus applies.

Qualifications

• Active Registered Nurse (RN) license in the state you work.
• One year of RN experience with clinical care delivery.
• Current driver’s license and ability to travel to patient homes (~20%).

Responsibilities

• Deliver skilled, patient-centered home health care in the patient’s home.
• Perform start-of-care assessments, including clinical evaluation and care planning.
• Educate patients and families about the admission process, diagnoses, and plans of care.
• Coordinate admissions with an interdisciplinary team and ensure timely EMR documentation.
• Initiate evaluations to other disciplines when indicated.
• Work independently, but with team support, and travel to patient homes.

We are hiring a Weekend Admissions Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Canonsburg, PA area. What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders
• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one
• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but never alone
• Travel to patient homes within assigned territory

Admission RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
