Qualifications

• Active RN license in the state you work.
• One year of RN experience.
• Due to field-based home health role, valid driver’s license and ability to travel ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting.
• Conduct start-of-care assessments and develop initial care plans.
• Educate patients and families through the admission process.
• Coordinate admissions with EMR documentation and interdisciplinary team.
• Travel to patient homes within assigned territory.
