Qualifications

• Active Registered Nurse license in the state you work.
• One year of RN experience and clinical care delivery.
• Valid driver’s license; 20% travel required.

Responsibilities

• Deliver skilled, patient-centered home health care with interdisciplinary team support.
• Perform patient assessments, medication management, and care planning.
• Educate families on disease management and treatment plans.
• Ensure compliant EMR documentation and safe care.
• Travel to patient homes within assigned territory.
