Heart to Heart Home Care is seeking professional, compassionate Direct Support Professionals who are enthusiastic about making a positive difference in the lives of individuals with Intellectual and Developmental Disabilities. Weekend Direct Support Professionals are needed in the Toms River area.

Shifts include Saturday and Sunday 9a-3p or Friday and Saturday 3p-11p, with locations in Toms River, Brick and Bayville.

Qualifications

• Non Probationary Driver's License and second form of ID
• Physical and PPD (within the last year)
• High School Diploma or Equivalent

Responsibilities

• Direct Support to individuals with Intellectual and Developmental Disabilities in a Group Home setting
• Provide support, transportation and assistance to enable daily routines
• Encourage independence through skill training in self-care, household chores and social/recreation activities

Job Description

Direct Support to individuals with Intellectual and Developmental Disabilities in a Group Home setting. Provide support, transportation and assistance for each individual to enable her/him to complete daily routines in a safe, healthy, productive and dignified manner. This usually includes toileting, bathing, dressing, laundry, supervision of recreation. Encourages independence by providing both the opportunity and the appropriate levels of skill training for the individuals in categories such as self-care, household chores, social skills, recreation skills, etc.
