Job summary

Qualifications

• High School Diploma or GED (or equivalent).
• Must be at least 18 years old.
• Valid driver’s license in good standing.
• Reliable transportation.
• Must pass background checks.
• Health screening (TB) as required.

Responsibilities

• Assist individuals with goal completion per support plan.
• Help with Activities of Daily Living (ADLs) based on needs.
• Safely transport individuals in company or personal vehicles.
• Accompany individuals to community activities.
• Complete required documentation in the EHR timely.
• Engage with individuals in home and community.
• Uphold rights and confidentiality; maintain HIPAA compliance.
• Follow policies and maintain professional boundaries.
• Report any abuse, neglect, or exploitation immediately.
• Administer prescribed medications after training.
• Be proficient with technology; stay awake during shifts.
• Perform physical tasks as required (lifting, transfers).
