COORDINATED CARE service inc seeks a Direct Support Professional for weekend shifts in Rochester, NY. The role focuses on safety, supervision, and assisting individuals with daily activities within a residential setting.

Requirements include a High School Diploma or GED and a valid NYS driver’s license. The position offers a $500 signing bonus, with half paid after 6 months and half after one year, and a schedule of Saturdays and Sundays from 1pm to 11pm.

Qualifications

• High School Diploma or GED is required.
• Valid NYS driver’s license is required.
• Ability to obtain and maintain Medical Certification.
• Experience with special needs population preferred.
• Strong verbal and written communication skills.

Responsibilities

• Provide safety and supervision to residents per agency policies.
• Assist residents with planning and accessing activities and community resources.
• Maintain required documentation and communicate with staff regularly.
• Participate in team meetings and contribute to person-centered plans.
• Respond to emergencies per procedures and assist with crisis management.
