Job summary

Triangle seeks a Direct Care worker at Green Street to provide direct care support on Saturdays and Sundays, 10am-6pm, in a residential setting. This part-time position assists individuals in achieving growth and maintaining daily living routines.

Responsibilities include documenting progress, following individualized plans, collaborating with the team, and attending supervision and trainings as directed. A high school diploma or GED is required.

Qualifications

• High school diploma or GED is required.
• Ability to communicate clearly in written and spoken forms.
• Ability to respond to inquiries from regulatory agencies or family members.

Responsibilities

• Provide direct care coverage and support during assigned shifts.
• Assist individuals with ISPs and growth areas identified in plans.
• Support in problem solving, personal care, medical care, socialization and daily living activities.
• Assist in development and implementation of behavior support plans.
• Complete documentation (logs, progress notes, incident reports, medication charts).
• Transport individuals to appointments and community activities as needed.
• Attend supervision and staff meetings as scheduled and participate in trainings.
• Additional duties as assigned by the Program Manager.
• Accomplish all tasks as appropriate.
