Job summary

PACS is seeking a Certified Nursing Assistant to provide routine daily nursing care and services in accordance with each resident’s care plan. You will identify residents, document care, monitor conditions, and assist with meals and daily activities on a standard shift.

The role emphasizes privacy, safety, and following established policies. CPR is preferred; the facility supports patient-centered care in a structured post-acute setting.

Qualifications

• Must possess at least a 10th grade education.
• Must be a licensed Certified Nursing Assistant per state laws; CPR preferred.
• Ability to read, analyze, and interpret general business materials.

Responsibilities

• Identify residents before treatments using wristband or photo ID.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain and update flow sheets and care notes accurately.
• Assist residents with daily activities, meals, and personal care.
• Protect resident privacy and confidentiality of information.
• Follow safety, infection control, and facility policies consistently.
