Bear River Behavioral Health Services is seeking a part-time weekend Case Manager to provide immediate crisis support for adults, coordinate rapid care, and connect clients to medical and community resources. The role includes documentation, collaboration with a multidisciplinary team, and ensuring safe transitions to lower levels of care.

Responsibilities include client transport, light meal prep, and ancillary duties as needed, with weekend hours (Sat & Sun).

Formación

• Bachelor's degree in psychology, social work, or related field.
• Utah licensure required or preferred.
• Experience with mental health or substance use services is a plus.

Responsabilidades

• Provide immediate, short-term care and recovery services for adults in crisis.
• Coordinate rapid care and de-escalation efforts.
• Support Receiving Center clients and connect them to community resources.
• Assist with transportation, light meal prep, and laundry as needed.
• Complete documentation and maintain service plans.
