Job summary

Senior Care Solutions is seeking a dependable Clinical Team Support for weekend shifts (Saturday, Sunday & Monday) to coordinate patient visits, manage clinician schedules, and support the Intake Department. Qualifications

• High school diploma or equivalent required.
• Previous scheduling, intake, medical office, healthcare coordination, or home health experience.
• Medicare Home Health experience strongly preferred.
• Excellent organizational, multitasking, and time management skills.
• Strong communication and customer service abilities.
• Ability to work efficiently in a fast-paced environment while managing multiple priorities.
• Proficiency with Microsoft Office and electronic medical record (EMR) systems.
• Experience with WellSky/Kinnser is a plus.

Responsibilities

• Coordinate and schedule patient visits in a fast-paced weekend environment.
• Communicate with field clinicians regarding schedules, changes, and patient needs.
• Support weekend on-call operations and urgent scheduling requests.
• Process referrals and intake documentation; support admissions as needed.
• Coordinate same-day and priority visits to ensure continuity of care.
• Collaborate with Clinical Managers and intake staff to optimize scheduling.
• Maintain accurate scheduling and patient data in the EMR.
• Escalate urgent clinical or operational issues to leadership.
