Job summary

PACS Group is seeking a dedicated caregiver to provide routine daily care to residents per signed assessments and care plans under supervision. You will use PCC to verify resident information during tasks such as meals and ALDS, and maintain clear, descriptive notes and incident reports.

You will protect resident privacy, report changes promptly, and assist with daily activities, room readiness, and mobility while fostering a warm, respectful environment on every shift.

Responsibilities

• Provide routine daily care and services per resident assessment and care plan under supervision.
• Use PCC to ensure accurate resident information when assisting with ALDS, meals and related tasks.
• Record entries on notes, SPAs and incident reports in a clear, descriptive manner.
• Report changes in residents' condition to Health and Wellness Director as soon as practical.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
• Assist residents with daily functions and mobility, prepare rooms, and escort residents to their rooms.
• Greet residents, introduce roommates, and create a warm, calm environment on the unit.
• Attend nursing reports and cooperate with facility staff to ensure continuity of services.
