Job summary

Texas Children's Hospital is seeking a Care Coordinator to collaboratively assess, plan, implement, monitor, and evaluate a patient’s health needs. You will coordinate comprehensive case management across care settings, partner with physicians, and optimize resource use to support high-quality outcomes.

Respiratory fit testing will be required for new hires. Key responsibilities include risk-based care planning, ensuring accurate documentation, and guiding patient/family through the care

Qualifications

• Bachelor's degree in nursing or associate's in nursing with BSN enrollment.
• Current RN licensure by the Texas Board of Nursing or Nursing licensure compact.
• BLS Certification from AHA.
• At least 3 years of nursing experience in an acute care setting.
• Experience in community health, complex pediatric patient care, home care, case management, managed care, or utilization review preferred.

Responsibilities

• Assess, develop, implement, and monitor a comprehensive plan of care with an interdisciplinary team and patient/family.
• Review clinical documentation and collaborate with medical staff to ensure appropriate level of care and patient placement.
• Identify problems and discuss with the team to mitigate barriers to care.
• Serve as content expert on level of care and ensure appropriate resource utilization.
• Coordinate timely transition of information to unit Care Coordinator to support revenue cycle.
• Complete patient visits across the continuum of care as indicated.
• Participate in orientation and training of new department members.
• Identify and monitor quality improvement opportunities in care delivery.
• Report quality, safety, and service issues to Quality Management.
