Job summary
AccentCare Inc. is seeking an RN Weekend Baylor for Home Health in Brooklyn Park, MN. This full-time role (36hrs) covers Golden Valley/Edina/Richfield with a Saturday to Monday 12-hour shift schedule.
The RN will ensure patient care plans are followed, supervise CHHAs and CNAs, and coordinate care across pediatric to geriatric patients. Qualified candidates will have an active RN license in the state, at least 1 year of RN experience, CPR certification, and a valid driver license with insured
Qualifications
- RN with current license in the state of operation.
- 1 year RN experience.
- Geriatric Home Health and OASIS experience required.
- CPR certification required.
- Must have a valid driver license with insured vehicle.
- Willingness to travel to all locations.
Responsibilities
- Assess patient status and implement the individualized care plan.
- Provide direct and indirect patient care to pediatric, adolescent, adult and geriatric patients.
- Perform initial nursing evaluation visit.
- Re-evaluate nursing needs and revise the care plan as required.
- Promote and maintain optimal client care; diagnose and treat health problems.
- Supervise and teach other clinical personnel with supervision responsibilities.
- Inform physician of changes in the client condition and needs; counsel client and family.
