Job summary

AccentCare Inc. is seeking an RN Weekend Baylor for Home Health in Brooklyn Park, MN. This full-time role (36hrs) covers Golden Valley/Edina/Richfield with a Saturday to Monday 12-hour shift schedule.

The RN will ensure patient care plans are followed, supervise CHHAs and CNAs, and coordinate care across pediatric to geriatric patients. Qualified candidates will have an active RN license in the state, at least 1 year of RN experience, CPR certification, and a valid driver license with insured

Qualifications

• RN with current license in the state of operation.
• 1 year RN experience.
• Geriatric Home Health and OASIS experience required.
• CPR certification required.
• Must have a valid driver license with insured vehicle.
• Willingness to travel to all locations.

Responsibilities

• Assess patient status and implement the individualized care plan.
• Provide direct and indirect patient care to pediatric, adolescent, adult and geriatric patients.
• Perform initial nursing evaluation visit.
• Re-evaluate nursing needs and revise the care plan as required.
• Promote and maintain optimal client care; diagnose and treat health problems.
• Supervise and teach other clinical personnel with supervision responsibilities.
• Inform physician of changes in the client condition and needs; counsel client and family.

What You Need to Know

The RN Weekend - Baylor is responsible for maintaining the delivery of quality patient care by assuring that the Patient Care Plan is followed. The position combines the activities of direct and indirect patient care and provides direct care to pediatric, adolescent, adult and geriatric patients. The Staff Nurse directs CHHAs and CNAs in patient care tasks, performs patient assessments, develops patient care plans, evaluates the plan effectiveness, modifies it as appropriate and documents all patient care provided.

If you meet these qualifications, we would love to meet you:

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• One (1) year experience as an RN.
• Licensed to practice as a registered nurse in the state of agency operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
• Must have Geriatric Home Health and OASIS experience.

Responsibilities

As a RN Weekend - Baylor, you will:

• Assessment of the status of nursing needs of assigned patients, implements the individualized patient care plan, evaluates the effectiveness of the care plan, and modifies as appropriate.
• Responsible for promoting and maintaining optimal client care. Diagnosing and providing treatment for human responses to actual or potential health problems.
• Makes the initial nursing evaluation visit.
• Re-evaluates the client's nursing needs as required.
• Initiates the plan of care and necessary revisions.
• Provides those services requiring substantial special skills within nurses scope of Practice Act.
• Assigns, supervises and teaches other clinical personnel, including written para-professional orders and supervision.
• Initiates appropriate preventative and rehabilitative nursing procedures.
• Informs the physician and other personnel of changes in the client's condition and needs.Counsels and instructs the client and family in meeting nursing and related needs
