Job summary

AccentCare is seeking an RN Weekend Baylor in Brooklyn Park, MN. The role supports home health and requires weekend shifts (Sat-Mon, 12-hr). The nurse ensures delivery of quality patient care, managing care plans and supervising CHHAs and CNAs.

Qualifications include RN licensure in the operating state, CPR, a year of RN experience, and a valid driver's license with travel readiness. Geriatric Home Health and OASIS experience are preferred.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the operating state.
• One year experience as an RN.
• Must possess and maintain CPR certification while employed in a clinical role.
• Must have a valid driver's license with an automobile that is insured and able to travel to all locations.
• Must have Geriatric Home Health and OASIS experience.

Responsibilities

• Assess nursing needs of assigned patients and implement and revise the care plan as appropriate.
• Promote and maintain optimal client care; diagnose and treat health problems.
• Make the initial nursing evaluation visit and re-evaluate as needed.
• Initiate and revise the plan of care.
• Provide services within the nursing scope of Practice Act.
• Supervise and teach other clinical personnel.
• Inform physicians and others of changes in patient condition.
