Job summary

Aspire Home Healthcare of Ohio is seeking a Weekend Admissions Registered Nurse (RN) to cover the Columbus area. You will handle admissions and SOC assessments, document histories, and coordinate care with the care team in a Friday–Monday schedule.

Ideal candidates are Ohio-licensed RNs with 1+ year of nursing experience (home health preferred), OASIS experience, and proficiency with WellSky (Kinnser). A CPR certification, valid driver's license, and HIPAA awareness are required.

Qualifications

Current Ohio RN license.
Minimum of one year nursing experience; home health preferred.
OASIS documentation experience strongly preferred.
Experience with WellSky (Kinnser) preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication and patient education abilities.
Current CPR certification.
Valid driver’s license and reliable transportation.
TB test and physical per agency requirements.

Responsibilities

Complete home health admissions and SOC assessments per physician orders and policies.
Obtain and document comprehensive medical histories.
Perform physical assessments including vital signs and mobility.
Complete OASIS documentation accurately and timely.
Develop individualized plans of care per physician orders.
Educate patients and caregivers on diagnoses, medications, and services.
Coordinate care with physicians, therapists, social workers, and aides.
