Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for Home Health Admission in Cuyahoga Falls, Ohio. This Full Time Weekend Admissions role serves adult and geriatric clients on a per-visit basis across Northeast Ohio, with weekend schedules Thursday-Sunday or Friday-Monday. You will follow care plans, perform medications and wound care, and document care using EMR.

Home health experience is required and candidate must hold an Ohio RN license. Travel to cases is expected as assigned.

Qualifications

• A valid OH RN license with ability to practice home health.
• Minimum one year of recent clinical/home health experience.
• Strong patient care and documentation skills.
• Able to travel to cases in designated areas.

Responsibilities

• Follow a designated home health care plan and visit patients.
• Administer medications, wound care, and treatments.
• Monitor patient conditions and report changes to supervisor.
• Document observations and care via EMR.
• Coordinate with clinical and support staff for patient care.
