Aspire Home Healthcare of Ohio is seeking a Weekend Admissions Registered Nurse (RN) to cover the Columbus area. You will handle admissions and SOC assessments, document histories, and coordinate care with the care team in a Friday–Monday schedule.

Ideal candidates are Ohio-licensed RNs with 1+ year of nursing experience (home health preferred), OASIS experience, and proficiency with WellSky (Kinnser). A CPR certification, valid driver's license, and HIPAA awareness are required.

Qualifications

• Current Ohio RN license.
• Minimum of one year nursing experience; home health preferred.
• OASIS documentation experience strongly preferred.
• Experience with WellSky (Kinnser) preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• TB test and physical per agency requirements.

Responsibilities

• Complete home health admissions and SOC assessments per physician orders and policies.
• Obtain and document comprehensive medical histories.
• Perform physical assessments including vital signs and mobility.
• Complete OASIS documentation accurately and timely.
• Develop individualized plans of care per physician orders.
• Educate patients and caregivers on diagnoses, medications, and services.
• Coordinate care with physicians, therapists, social workers, and aides.
