Aspire Home Healthcare of Ohio in Columbus is seeking a Weekend Admissions Registered Nurse (RN) to perform Start of Care assessments, develop plans of care, and coordinate services for home health patients Fri–Mon.

Qualifications

• Current and active Ohio RN license.
• Minimum of one year nursing experience; home health experience preferred.
• OASIS documentation experience preferred.
• CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete home health admissions and Start of Care assessments.
• Document comprehensive medical histories and vital signs.
• Develop individualized plans of care based on physician orders.
• Coordinate care with physicians, therapists, social workers, and home health aides.
• Ensure HIPAA compliance and accurate clinical documentation.
• Identify changes in patient condition and coordinate referrals as needed.
