This position serves as the primary admissions nurse Friday through Monday.Key Responsibilities• Complete home health admissions and Start of Care (SOC) assessments in accordance with physician orders and agency policies• Obtain and document comprehensive medical histories• Perform physical assessments, including vital signs, mobility evaluations, and safety assessments• Complete OASIS documentation accurately and within required timeframes• Develop individualized plans of care based on patient needs and physician orders• Educate patients and caregivers regarding diagnoses, medications, treatment plans, and home health services• Coordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team members• Ensure compliance with all federal, state, and agency regulations, including HIPAA requirements• Identify and communicate changes in patient condition to the appropriate healthcare providers• Facilitate referrals for additional home health services as needed• Maintain timely and accurate clinical documentationQualificationsQualifications• Current and active Registered Nurse (RN) license in the State of Ohio• Minimum of one year of nursing experience required; home health experience preferred• OASIS documentation experience strongly preferred• Experience with WellSky (Kinnser) preferred• Strong clinical assessment and critical thinking skills• Excellent communication and patient education abilities• Current CPR certification• Valid driver's license, reliable transportation, and automobile insurance• Current TB test and physical examination in accordance with agency requirements• Ability to attend orientation and training at the Aspire Home Healthcare of Ohio officeJoin Aspire Home Healthcare of Ohio and make a meaningful impact by helping patients receive quality care where they are most comfortable—at home.
