Qualifications
- Experience in healthcare admissions, skilled nursing, case management, marketing, or customer service.
- Strong communication and interpersonal abilities are essential.
- Ability to multitask, organize, and operate independently.
Responsibilities
- Coordinate and facilitate all weekend admissions and transfers.
- Review referral information for completeness and appropriateness.
- Communicate with hospitals, case managers, discharge planners, physicians, and referral sources.
- Verify admission documents and ensure regulatory compliance.
- Complete admission paperwork and obtain signatures.
- Welcome residents and families with a positive admission experience.
- Coordinate with nursing, therapy, dietary, social services, and other departments.
- Assist with insurance verification and collect demographic and financial info.
- Maintain admission logs, census records, and referral tracking.
- Respond promptly to referral inquiries and admission requests.
- Conduct facility tours for prospective residents and families.
- Assist with marketing and relationship-building with referral sources.
- Support weekend census management and bed availability tracking.
- Ensure admissions comply with policies, state regulations, and federal guidelines.
- Support the facility’s mission of resident-centered care.
