CareAparent is seeking a Weekend Admissions Nurse (RN) to perform in-home assessments, care plan development, and medication set-ups. The role requires traveling across the Twin Cities metro to visit clients and coordinate with interdisciplinary teams.

You will educate patients and families and ensure proper documentation in the EMR. The position emphasizes delivering compassionate, high-quality home health care and maintaining a patient-centered approach while supporting a growing, stable

Qualifications

• Current Minnesota RN license.
• Experience with seniors and client assessments.

Responsibilities

• Conduct initial home health care OASIS assessments for Starts of Care, Resumptions of Care and Recertifications.
• Develop and monitor plan of care based on physician orders and client assessment.
• Perform medication set-ups and nursing tasks as assigned.
• Coordinate referrals to physical, occupational, speech and other therapy teams.
• Provide clinical oversight and leadership to licensed and unlicensed staff.
• Prepare visit reports, patient summaries and lead team conferences.

The Weekend Admissions Nurse (RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and completing clinical documentation. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care.
