CareAparent is seeking a Weekend Admissions Nurse (RN) to perform in-home assessments and develop patient care plans on a Friday–Sunday schedule. The role includes medication setup, clinical documentation, and collaboration with a multidisciplinary team to ensure high-quality home health care for seniors in the Twin Cities.

The ideal candidate holds a Minnesota RN license, strong communication skills, and willingness to travel to client homes.

Responsibilities

• Conduct initial home health care OASIS assessments (Starts of Care, Resumptions, Recertifications).
• Develop and monitor plan of care based on physician orders and client assessments.
• Coordinate with interdisciplinary teams (PT, OT, Speech) and supervision by Clinical Manager.
• Prepare visit reports and patient records summaries; lead care team conference.

The Weekend Admissions Nurse (RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and completing clinical documentation. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Provide compassionate care to seniors who need it most.
