Job summary

CareAparent is seeking a Weekend Admissions Nurse (RN) to perform in-home assessments, re-evaluations and care plan preparation Friday through Sunday, 8:00 am to 5:00 pm. You will educate patients and families, coordinate with clinical teams, and ensure compliant documentation in the EMR.

Travel across the Twin Cities may be required to visit clients in their homes. The role emphasizes compassionate, high-quality care, starts with Starts of Care and Recertifications, and offers competitive pay

Qualifications

• Current Minnesota RN license.

Responsibilities

• Conduct initial and ongoing in-home OASIS assessments.
• Develop and update plans of care based on physician orders.

The Weekend Admissions Nurse (RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and completing clinical documentation. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care.
