CareAparent is hiring a Registered Nurse Case Manager (RNCM) to perform in-home assessments, re-evaluations, and care plan development for seniors in the Minneapolis area. You will educate families and providers, supervise HHA/CNAs, and ensure high-quality home health care delivery.

Responsibilities include Starts of Care, Recertifications, and coordination with interdisciplinary teams. Travel to client homes is required; on-call weekends/holidays may be needed.

Qualificações

• Current Minnesota RN license.
• Experience with seniors and client assessments.
• Excellent customer relations and interpersonal skills.
• Excellent verbal/written communication skills.
• Travel to client homes as needed.
• On-call availability for weekends and holidays.
• Valid driver’s license with insured vehicle.

Responsabilidades

• Conduct initial home health care OASIS assessments for Starts of Care, Resumptions of Care, and Recertifications.
• Work within primary zone (local area), covering other zones as needed.
• Develop a plan of care based on physician orders and client assessment, and update as condition changes.
• Facilitate referrals to interdisciplinary teams (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention).
• Perform skilled nursing tasks in coordination with Clinical Manager and scheduling.
• Oversee and supervise LPN visits and ensure plan of care is followed.
• Provide clinical oversight, delegation, and leadership to licensed clinicians and unlicensed care team members.
• Prepare visit/shift reports, patient records summaries, and lead care team case conferences.
• Coordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followed.
• Report on changes in client condition to physicians and Clinical Manager.
• Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms.

Registered Nurse Case Manager (RNCM) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising HHA's and CNAs. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Pay range: $37.50 - $46.00/hour

Duties & Responsibilities

• Prepare visit/shift reports, patient records summaries, and lead care team case conferences with the care team.
• Coordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followed.

• Ok with travel to and from client homes
• Ability to work on call for rotating weekends, and holidays
• Possess a valid driver's license and insured personal vehicle
