Job summary

CareAparent is seeking a Weekend Admissions Nurse (RN) to provide in-home assessments and care planning on a Fri-Sun schedule (8:00 am–5:00 pm). You will conduct Starts of Care, Recertifications, and collaborate with therapists and physicians to ensure safe, high‑quality home health care for our clients.

You will educate patients and families, document thoroughly in CareAparent EMR, and support a multidisciplinary team while traveling across the Twin Cities metro.

Qualifications

• Current Minnesota RN license.
• Previous experience with seniors and client assessments.
• Excellent customer relations and interpersonal skills.
• Excellent verbal/written communication skills.
• Ok with travel to and from client homes.
• Ability to work on call for some holidays.
• Possess a valid driver's license and insured personal vehicle.

Responsibilities

• Conduct initial home health care OASIS assessments for Starts of Care, Resumptions of Care, and Recertifications.
• Develop a plan of care based on physician orders and initial client assessment; monitor and revise as needed.
• Facilitate referrals to interdisciplinary teams (PT, OT, Speech) and Falls Prevention.
• Perform skilled nursing tasks in coordination with the Clinical Manager and scheduling.
• Provide clinical oversight and leadership to licensed and unlicensed staff.
• Prepare visit reports, patient summaries, and lead care team conferences.
• Report changes in client condition to physicians and the Clinical Manager.
• Document assessments and visits using CareAparent EMR platforms.
• Attend monthly nurse meetings with the Clinical Team.
