Job summary

CareAparent is seeking a Weekend Admissions Nurse (RN) to perform in-home assessments, re-evaluations and care plan preparation Friday through Sunday, 8:00 am to 5:00 pm. You will educate patients and families, coordinate with clinical teams, and ensure compliant documentation in the EMR.

Travel across the Twin Cities may be required to visit clients in their homes. The role emphasizes compassionate, high-quality care, starts with Starts of Care and Recertifications, and offers competitive pay

Qualifications

• Current Minnesota RN license.
• Experience conducting home health assessments.
• Ability to plan and monitor care per physician orders.
• Willingness to travel to client homes in the Twin Cities.

Responsibilities

• Conduct initial and ongoing in-home OASIS assessments.
• Develop and update plans of care based on physician orders.
• Coordinate referrals with physical, occupational, aroma, and speech therapy.
• Provide clinical oversight and leadership to licensed and unlicensed staff.
• Document visits and prepare patient records in the EMR.
