Job summary

CareAparent is hiring a Registered Nurse to conduct in-home assessments, establish and adjust care plans, and supervise LPNs and CNAs. The role supports seniors at home across three zones with travel as needed.

A Minnesota RN license, valid driver's license, and at least one year of home health experience are required, with BSN preferred for advancement. You will work under the Clinical Manager, ensure adherence to CMS and Joint Commission standards, and participate in ongoing training.

Qualifications

• RN degree is required from an accredited nursing program.
• BSN preferred for this role.
• Minimum of 1 year of relevant home care experience.

Responsibilities

• Perform initial home health care OASIS assessments including Starts of Care, Resumptions of Care, and Recertifications.
• Develop and revise plan of care based on physician orders and patient assessments.
• Oversee LPN visits and ensure plan of care is followed; report to Case Manager.
• Coordinate and supervise CNA/PCA/HHA visits and ensure care adherence.
• Document client assessments and visits in the EMR and communicate changes to physicians and case management.
• Be available for on-call coverage during evenings and weekends.

The Start of Care/Resumption of Care Registered Nurse (SOC/ROC RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising HHA's and CNAs. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care.
