Job summary

CareAparent is seeking a Weekend Admissions Nurse (RN) to perform in-home assessments and re-evaluations, develop care plans, set up medications, and complete clinical documentation. The role requires educating patients, families, and other providers to safely perform care, helping clients stay at home with comfort and dignity.

The position offers a 8:00 am to 5:00 pm schedule on Fri–Sun with a weekly 24 hours (0.6 FTE) and pay range $41–$46 per hour, plus benefits.

Qualifications

• Must have Minnesota RN license and current certification.
• Experience with seniors and in-home client assessments.
• Excellent communication and customer relations skills.
• Willingness to travel to client homes.
• Available to work weekends/holidays on call.

Responsibilities

• Conduct initial OASIS assessments and plan of care for starts of care/recertifications.
• Ability to drive throughout the Twin Cities metro for visits.
• Develop and monitor plan of care based on physician orders and client assessments.
• Coordinate referrals to Physical, Occupational, Aroma, Speech Therapy and Falls Prevention.
• Perform skilled nursing tasks in coordination with Clinical Manager and scheduling.
• Provide clinical oversight and leadership to licensed and unlicensed staff.
• Prepare visit/shift reports and care team conference summaries.
• Report changes in client condition to physicians and Clinical Manager.
• Document assessments and visits in CareAparent EMR.
• Attend monthly nurse meetings with Clinical Team.

We are currently seeking a Weekend Admissions Nurse (RN) on a schedule of Friday through Sunday (8:00 am to 5:00 pm).

At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. The Weekend Admissions Nurse (RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and completing clinical documentation. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Provide compassionate care to seniors who need it most. Your services help clients stay in their home, maintain a comfortable lifestyle, and live happier lives.

Pay range: $41.00 - $46.00/hour

Weekly Hours: 24 (.6 FTE)

Schedule: 8:00 am to 5:00 pm, Friday - Sunday

Duties & Responsibilities

• Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications
• Ability to drive throughout the twin cities metro for visits and clients
• Develop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of condition.
• Facilitate referrals to interdisciplinary teams (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention).
• Perform skilled nursing tasks as assigned in coordination with the Clinical Manager and CareAparent scheduling.
• Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed care team members.
• Prepare visit/shift reports, patient records summaries, and lead care team case conferences with the care team.
• Report on changes in client condition to physicians and Clinical Manager.
• Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms.
• Attend monthly in-person or virtual nurse meetings with the Clinical Team

Qualifications

• Current Minnesota RN license
• Previous experience with seniors and client assessments
• Excellent customer relations and interpersonal skills
• Excellent verbal/written communication skills
• Ok with travel to and from client homes
• Ability to work on call for some holidays
• Possess a valid driver's license and insured personal vehicle
