Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to cover the Petaluma area. You will perform Start of Care admissions, conduct follow-up visits, and manage home visits with autonomy in scheduling.

Candidates must hold a CA RN license and BLS/CPR certification, with experience in SOC/OAS, high-acuity patients, and IV therapies. Mileage reimbursement, company iPad, and robust benefits are included.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Previous experience with IV Medication infusions highly preferred.
• Proficiencies initiating intravenous (IV) therapy is a plus.

Responsibilities

• Conduct follow-up and evaluation visits in designated territory.
• Make home visits within a 5-15 mile radius.
• Reconcile medications and perform wound care when needed.
• Monitor patient conditions and report changes to clinical staff.

• Initiate and administer IV infusions per protocol.
• Manage administrative tasks with dedicated resources.

Part time Registered Nurse Case Manager

Anticipated Territory : Petaluma Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks
• Flexible work schedules: Create your own schedule and hours
• Targeted territories. Typically patient visits are within a 5-15 mile radius.
• Autonomy: No clocking in/out
