Job summary

21st Century Home Health Services (21HHS) is seeking a Part Time Registered Nurse Case Manager to cover the Livermore area. The role requires delivering patient-centered home health care, performing SOC assessments, and coordinating with the clinical team to ensure optimal outcomes.

Weekend availability (Saturday and Sunday) is required, with potential for additional weekday shifts. You will drive patient visits within a 5–15 mile radius, autonomously manage care plans, and participate in our

Part time Registered Nurse Case Manager

Anticipated Territory : Livermore area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks
• Flexible work schedules: Create your own schedule and hours
• Targeted territories. Typically patient visits are within a 5-15 mile radius.
• Autonomy: No clocking in/out

• Previous experience with IV Medication infusions highly preferred
