Job summary

21st Century Home Health Services seeks a part-time Registered Nurse Case Manager for the Santa Rosa area. You will assess patients in their homes, develop care plans, and coordinate with physicians and the care team to deliver high-quality home health services.

Responsibilities include SOC/OASIS assessment, IV infusions, medication reconciliation, and patient education. CA RN license required; BLS/CPR; valid CA driver’s license; flexible weekend availability.

Qualifications

• CA RN license in good standing required.
• BLS/CPR certification required.
• SOC/OASIS experience highly desired.
• IV medication infusions experience preferred.

Responsibilities

• Perform Start of Care (SOC) admissions and OASIS assessments.
• Conduct follow-ups and evaluation visits in designated territory.
• Make home visits to patients in specified area.
• Reconcile medications and perform wound care and other procedures.
• Monitor patients' conditions and report changes to the clinical team.
• Collaborate with physicians and other staff to ensure quality care.
• Document observations, interventions and evaluations of patient care.
• Initiate and administer IV antibiotics and diuretics per protocol.

Part time Registered Nurse Case Manager

Anticipated Territory : Santa Rosa Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

To be a success:

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
• High level of clinical knowledge in home health care is highly preferredExperience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
• Previous experience with IV Medication infusions highly preferred
