21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager for the Novato area. You will assess patients for home care needs, perform SOC admissions and follow-up visits, and coordinate care with physicians and the clinical team.

Schedule options include weekend days (Sat/Sun) with possible additional weekdays; autonomy and flexible planning are offered, along with mileage reimbursement, an iPad, and comprehensive Benefits.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• SOC/OASIS experience is highly desired.
• High level of clinical knowledge in home health care.
• Experience with IV medication infusions preferred.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Perform follow-ups and evaluation visits in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments and procedures.
• Monitor clients’ conditions and report changes to the Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Document observations, interventions, and evaluations for patient care management.
• Initiate and administer IV medication infusions safely per protocol.

Part time Registered Nurse Case Manager

Anticipated Territory : Novato Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

To be a success:

• Current and unrestricted CA RN license

• High level of clinical knowledge in home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
• Previous experience with IV Medication infusions highly preferred
• Proficienciesinitiating intravenous (IV) therapy is a plus
