Job summary

21st Century Home Health Services is seeking a part-time Registered Nurse Case Manager to cover the Sonoma Area. You will conduct SOC admissions, perform follow-ups, and visit patients at home within a 5–15 mile radius, delivering high-quality nursing care.

The role requires CA RN license, BLS/CPR, and a valid CA driver’s license. You’ll work with an interdisciplinary team, coordinate care, and have flexible weekend scheduling with opportunities for advancement.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Follow up and evaluation visits as needed in designated territory.
• Make home visits to patients in designated territory.
• Perform assigned duties including medication reconciliation, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Document observations, interventions, and evaluations pertaining to patient care management.
• Initiate and administer IV medication infusions safely per protocols.
• Provide concierge support for Case Managers to assist with administrative tasks.
• Create flexible work schedules and manage your own hours.
• Work within targeted territories with 5–15 mile patient visits radius.
• Operate with autonomy and no clocking in/out.
• Opportunities for advancement within multiple RN tiers and leadership roles.

Part time Registered Nurse Case Manager

Anticipated Territory: Sonoma Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks
• Flexible work schedules: Create your own schedule and hours
• Targeted territories. Typically patient visits are within a 5-15 mile radius.
• Autonomy: No clocking in/out
