Job summary

Assured Quality Homecare (ASQ) in Warwick, RI is seeking an experienced Per Diem Private Duty RN to provide clinical leadership for private-duty home care clients across Rhode Island. The role focuses on weekday business hours with consistent Monday–Friday availability.

You will perform in-home assessments, develop care plans, supervise CNAs, coordinate with physicians and families, and support transitions from hospital to home.

Qualifications

• Active, unrestricted Rhode Island RN license.
• Current CPR certification.
• Five or more years of RN experience.
• Five or more years of care planning, case management, or patient-navigation experience.
• Experience in home care, rehabilitation, long-term care, assisted living, hospice, or related setting preferred.
• Consistent weekday business-hour availability.
• Strong assessment, documentation, and communication skills.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to travel throughout Providence, Kent, and Washington counties.
• Ability to lift up to 50 pounds and assist with client mobility when required.

Responsibilities

• Conduct in-home assessments, admissions, and client intakes.
• Develop, review, and update individualized care plans.
• Supervise CNAs and provide client-specific instruction.
• Collaborate with physicians, families, hospitals, and other providers.
• Accompany clients to medical appointments when needed.
• Support hospital discharges and safe transitions home.
• Provide medication management, wound care, and other skilled nursing services.
• Document clinical updates accurately and communicate concerns promptly.
