Job summary

Assured Quality Homecare (ASQ) is hiring a Per Diem Private Duty RN/LPN to provide clinical leadership and oversee private-duty clients across Connecticut. The role is part-time with consistent weekday business hours, focusing on admissions, assessments, and ongoing clinical oversight in home settings.

You will perform in-home assessments, develop care plans, supervise CNAs, collaborate with physicians and families, and support hospital discharges.

Qualifications

• Active RN/LPN license.
• Current CPR certification.
• Five or more years of RN/LPN experience.
• Five or more years in care planning, case management, or patient-navigation.
• Experience in home care, rehab, long-term care, hospice, or related setting preferred.
• Weekday business-hour availability.
• Strong assessment, documentation, and communication skills.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to travel across Providence, Kent, and Washington counties.
• Ability to lift up to 50 pounds and assist with client mobility.

Responsibilities

Assured Quality Homecare (ASQ) is seeking an experienced Registered Nurse to provide clinical leadership and oversight for private-duty home care clients throughout Rhode Island.

This position is designed for a weekday clinical professional. Admissions, assessments, medical appointments, and provider collaboration primarily occur during regular business hours. This is not a night- or weekend-focused per diem role.

Your Role

As a Per Diem Private Duty RN/LPN, you will guide clients through the most important parts of their care journey—from the initial assessment to ongoing clinical oversight.
