Job summary

Assured Quality Homecare, LLC is seeking an experienced Registered Nurse or Licensed Practical Nurse for per diem private duty care in Rhode Island. The role emphasizes clinical leadership, in-home assessments, and ongoing oversight during weekday business hours.

Qualifications

• Active RN / LPN license.
• Current CPR certification.
• Five or more years of RN/LPN experience.
• Five or more years of care planning, case management, or patient-navigation experience.
• Experience in home care, rehabilitation, long-term care, assisted living, hospice, or related setting preferred.
• Consistent weekday business-hour availability.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to travel throughout Providence, Kent, and Washington counties.
• Ability to lift up to 50 pounds and assist with client mobility.

Responsibilities

• Conduct in-home assessments, admissions, and client intakes.
• Develop, review, and update individualized care plans.
• Supervise CNAs and provide client-specific instruction.
• Collaborate with physicians, families, hospitals, and other providers.
• Accompany clients to medical appointments when needed.
• Support hospital discharges and safe transitions home.
• Provide medication management, wound care, and other skilled nursing services.
• Document clinical updates accurately and communicate concerns promptly.
