Job summary

Assured Quality Homecare (ASQ) is seeking a Per Diem Private Duty RN to provide clinical leadership for private-duty home care clients across Rhode Island. The role is weekdays, with in-home assessments, care plan oversight, and coordination with physicians. Per diem/part-time schedule emphasizes regular business hours.

The RN will supervise CNAs, manage medications, and support transitions home. Rhode Island RN license, CPR, and 5+ years of RN and care planning experience are expected.

Qualifications

• Active, unrestricted Rhode Island RN license.
• Current CPR certification.
• Five or more years of RN experience.
• Five or more years of care planning, case management, or patient-navigation experience.
• Experience in home care, rehabilitation, long-term care, or related setting preferred.
• Consistent weekday business-hour availability.

Responsibilities

• Conduct in-home assessments, admissions, and client intakes.
• Develop, review, and update individualized care plans.
• Supervise CNAs and provide client-specific instruction.
• Collaborate with physicians, families, hospitals, and other providers.
• Accompany clients to medical appointments when needed.
• Support hospital discharges and safe transitions home.
• Provide medication management, wound care, and other skilled nursing services.
• Document clinical updates accurately and communicate concerns promptly.
