Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Weekday On-Call Nurse (RN) to provide compassionate, patient-centered care during on-call hours. This role requires coordinating with the care team to implement care plans and ensure high-quality, individualized patient care.

The ideal candidate holds an RN license, ADN/AAS with BSN preferred, and CPR/FA certification. On-call schedule is Monday-Thursday 5pm-8:30am, with daily pay and flexible options.

Qualifications

• Education: ADN or AAS required; BSN preferred.
• Current RN license in the state of assignment.
• CPR/FA certification.
• Reliable transportation to care sites/work locations.
• 1 year of RN hospice or similar setting experience preferred.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and coordinate with providers to adjust care as needed.
