Job summary

AMEDISYS HOME HEALTH is hiring for a Hospice Weeknight On Call RN. Respond to after-hours telephone requests from our triage RN, providing skilled assessment and interventions, including possible after-hours admissions and end-of-life visits.

Schedule is Monday to Thursday, 5pm to 8am, four on-call nights. Salary ranges from $83,200 to $86,000 with mileage; benefits begin the first month and a comprehensive suite of health and wellness programs are offered.

Qualifications

• One+ year of nursing experience required.
• Current RN license, state-specific.
• Current CPR certification required.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the total plan of care with continuity across the care team.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families.
