Alzheimer’s Community Care is seeking a Wandering Prevention Coordinator to safeguard patients with dementia and support families. You will maintain patient and caregiver files, ensure accurate data entry, and collaborate with Law Enforcement and SafetyNet Tracking Systems to improve safety and care coordination.

The role emphasizes enrollment coordination, appointment management, and documentation across EMR/ADS systems, with a focus on reliable communication and timely services for caregivers

Qualifications

• High school diploma or equivalent.
• Three years of professional administrative experience, preferably in a health-related field.
• Driver license and eligibility for benefits.
• Proficiency with MS Word/Excel and office equipment; adapt to new software.

Responsibilities

• Referral coordination with caregivers and enrollments within designated territories.
• Manage enrollment appointments at centers, offices, or patient homes.
• Handle service inquiries and follow-up calls with caregivers.
• Schedule monthly maintenance visits and document inventory.
• Maintain EMR/ADS and SafetyNet records with accurate demographics and notes.
• Track equipment, supplies, and process documents for team members.
• Coordinate searches for missing equipment with SafetyNet and caregivers.
