Alzheimer’s Community Care in Florida is seeking a Wandering Prevention Coordinator to help safeguard patients with Alzheimer’s disease and other cognitive disorders. The position emphasizes individualized solutions and education to enhance patient safety and caregiver support.

Qualifications

• High school diploma or equivalent.
• Three years of professional administrative experience, preferably in a health-related field.
• Valid driver license.

Responsibilities

• Coordinate referrals and enrollments with caregivers within territories.
• Schedule enrollment appointments at day centers, the Organization’s office, or in patients' homes.
• Handle inquiries about services and follow up with caregivers.
• Maintain monthly maintenance visits for equipment with caregivers.
• Update monthly worksheets and contribute to the ID Locator Master spreadsheet.
• Collaborate with SDS to ensure service delivery and inventory.
• Track supplies needed for service delivery.
• Enter patient and caregiver data into EMR/ADS and SafetyNet systems.
• Process enrollment, annual reviews, and communications to team members.
• Maintain tracking logs and address missing information.
• Coordinate searches for missing equipment with SafetyNet and caregivers.
